REGISTRATION INFORMATION

Registration for the 2019 FH Global Summit is open. Register early to take advantage of our best rates.

Registration fee covers all sessions, breakfast and lunch on Oct 21, as well as two evening networking receptions. We recommend all attendees plan on arriving by noon on Sunday 10/20 and leave after 7 pm on Monday 10/21 to take advantage of the full conference. Lodging must be booked separately (see below).

Nurse/Advanced Practice Providers*
Attendees who are Registered Nurses, Clinical Nurse Specialists, Nurse Practitioners, or Physician Assistants.

Allied Health Professional*
Attendee who belong to one of the following categories — Genetic Counselor, Pharmacist, Sonographer, Electrophysiology Specialist, Invasive Specialist, Vascular Specialist, Exercise Physiologist, Occupational Therapist, Physical Therapist, Clinical Psychologist, Clinical Social Worker, or Registered Dietician.

Fellow/Student*
Team Members in Training, including Nursing, Physician Assistant; Clinical Pharmacy Students; Medical Student or Medical

Individual with FH
An individual with FH or family member of someone with FH.

* VERIFICATION REQUIRED. These categories may be required to submit an email to confirm registration with proof of licensure, a business card, or a letter written on official letterhead and signed by a department supervisor verifying status.

FH GLOBAL SUMMIT HOTEL LODGING

Make sure to book your room through the FH Global Summit room block. We have secured a limited number of rooms and the hotel will sell out. Book early to ensure onsite lodging.

EVENT AND REFUND POLICY

The FH Global Summit is an invite-only event. All registrations are subject to our verification and approval, which we try to provide in 7-10 business days. Attendee badges are non-transferable. For the security of our attendees, all registrants must wear a valid FH Global Summit badge at all times.

A written notice of cancellation must be received by September 18, 2019 in order to receive a refund. Cancellations received prior to this date will be honored minus a $75 administration fee. If you cancel after September 18, 2019, you will forfeit all fees. Cancellation refunds will be processed within 30 days after the conclusion of the Conference.

By registering for the 2019 FH Global Summit, attendee understands and agrees that attendee’s name, city, and state may be shared with other attendees and exhibitors.

Any photos or other recordings taken by FH Foundation staff and their vendors at the FH Global Summit event may be used to promote the event and community in a variety of ways, such as on our website or in promotional materials.
